The connection between thyroid and infertility

The human body and its glands are interconnected to various functions. At times the thyroid gland can affect women's health,  especially when it is an underactive thyroid gland. This could lead to fertility issues as certain hormones cannot be produced with hypothyroidism.

When the thyroid levels are low, it will affect the release of an egg, which impairs fertility. Certain autoimmune or pituitary disorders, which are caused due to hypothyroidism, can also impair fertility.

Thyroid hormone levels

Thyroid-stimulating hormone (TSH) blood test helps to identify the thyroid hormone level. TSH needs to remain at an optimal level or would have an effect on ovulation. The thyroid hormone levels have a link with the development of the fetus. It is essential to keep the iodine level also optimal as thyroid hormones are synthesized from iodine. With the help of medications, doctors can help treat the variations in TSH or iodine levels. The TSH should not be higher than 2.5 and 3.0 at the time of pregnancy.

Hyperthyroidism - when the thyroid is overactive and excessive production of thyroid hormones can be considered to be hyperthyroidism. Graves’ disease and autoimmune diseases are common manifestations of this. By performing the TSH blood test, doctors can identify the presence of hyperthyroidism. Women tend to get irregular or infrequent periods, and men would have a low sperm count. Medications, radioactive iodine, or surgery are various ways to treat hyperthyroidism.

Hypothyroidism - when the thyroid is underactive and produces insufficient or fewer thyroid hormones, it can be termed as hypothyroidism. Lack of iodine in the diet or autoimmune disease can lead to hypothyroidism. Reduced fertility, higher chances for miscarriage, lower infant intelligence, pre-eclampsia, premature birth, and infant death are certain issues related to hypothyroidism. A simple blood test will help identify the problem, and the supplementation with iodine or thyroxine can help treat the problem.

Thyroid antibodies

Thyroid antibodies also have an impact on fertility. Thyroid antibodies are produced when the immune system attacks the thyroid gland by mistake. Infertile women would have the presence of thyroid antibodies in them. These antibodies lead to the risk of getting pregnant. Doctors recommend blood tests so that treatment could be initiated on time.

Changes in lifestyle and reducing stress can always have an impact on thyroid levels. Better diet and regular exercise can also help prevent and allow thyroid levels to be normal. At Srishti Assisted Facility & Advanced Laparoscopic Center, doctors can help treat problems with thyroid before IVF.

Do’s And Don’ts After An Embryo Transfer

Embryo transfer is the crucial last hurdle in the IVF cycle on which the entire success of the procedure stands. During the process, the best and viable embryos are transferred into the women's uterus and monitored for successful implantation and positive pregnancy. Usually, all the doctors follow a two weeks wait and watch approach to know if the pregnancy has occurred. This is the ultimate time that tests the patience of an individual. Although they are no restrictions imposed on the patient during this period, there are certain dos and don'ts they have to follow for a successful pregnancy during the waiting period.

DO'S

FOLLOW NORMAL ROUTINE – Patients are encouraged to perform normal routine activities after leaving the hospital. Too much of bed rest is not at all recommended.
NUTRITIOUS DIET – Eat a diet rich in proteins, iron, and fiber.avoid unhealthy junk food. Try to include more leafy vegetables and fruits that help in proper digestion. Also, try to avoid caffeine as it is associated with miscarriage.
FLUID INTAKE – It is recommended to keep the body hydrated and drink plenty of water and fruit juices. Proper hydration is required to prevent constipation that can be caused due to the intake of medicines.
MEDICATION – All the prescribed medications should be taken on time, and no dosage should be missed. It is suggested to continue the intake of folic acid and essential multivitamins.
CONSULT DOCTOR – In case of a medical emergency or any other health issues, it is best to consult the treating doctor rather than trying own remedies or over the counter medications.

DON'TS

STRENUOUS EXERCISE – Even tough women can perform their daily chores. It is advised not to perform any vigours exercise that disturbs the embryo position. A slow walk is always the best option to be considered.
EXTREME TEMPERATURE – It is best to avoid hot water baths or steam baths as any rise in body temperature will affect the growth of the embryo.
STRESS YOURSELF – Stress is directly proportional to the success rate of pregnancy. It is best to practice meditation and relaxation exercises to control stress levels. Also, keeping oneself engaged in daily routine will reduce the anxiety caused due to the waiting period.
SEARCH FOR SYMPTOMS – Don't engage yourself in internet gazing and search for pregnancy symptoms. Each individual's response to pregnancy will be different. Also, it is not advised to take a home pregnancy test during the waiting period.
KEEP HIGH EXPECTATIONS – Even though your just one step away from your dream to transform into reality, it is best to have neutral thoughts and stay calm and composed to the extent possible.

Benefits Of Frozen Embryo Transplantation

Frozen Embryo Transplantation

Frozen Embryo Transplantation is an assisted reproductive technology (ART) where a cryopreserved(which is preserved from previous IVF cycle)embryo is thawed and transplanted to the women's uterus. Having a frozen embryo removes the need to transfer an embryo during the same stimulated ovulation IVF cycle when a woman's eggs are retrieved. This added time, or delayed implantation, increases the chance of a successful pregnancy and birth. Studies from around the world have shown us that In Vitro Fertilization pregnancies following a frozen embryo transfer are more similar to natural conception pregnancies than the fresh embryo transfer cycles.

frozen embryo transfer

Below listed are few benefits of Frozen Embryo Transplantation.

  • Having a frozen embryo can stand as a substitute if your fresh embryo transfer fails. There is no need to repeat the stimulation cycle for the second time.
  • Reduces pregnancy risk of premature birth, low birth weight, and pre-eclampsia.

  • It is an easy procedure that does not require surgery or anesthesia.

  • Less medication.

  • Less stress.

  • Decreased miscarriage rates.

  • We can perform genetic screening that can increase the chances of a successful and healthy pregnancy.

  • Freezing all embryos can reduce the bleak effect of progesterone during pregnancy.

  • Reduces the risk of ovarian hyper stimulation syndrome(OHSS).

  • Patients can decide the day of their transfer months in advance, which will then be used to determine their cycle initiation date.

  • Less expensive method than a fresh IVF cycle.

Success Rates of Frozen Embryo Transfer

Frozen Embryo Transfer has a higher success rate comparing to fresh IVF as it is possible to optimize the lining of the uterus before implantation.

Intrauterine Insemination and Unexplained infertility

Most couple have a strong desire to bear a child at some point in their life. In normal cases, a couple can conceive within one year of having frequent or unprotected sex. When no results are seen within one year, the couple must go for fertility tests. After the tests, if no infertility issues are found in both the partners then it is referred to as “unexplained infertility”. In these cases, doctors often suggest the couple to have Timed sexual intercourse at the time of ovulation for few months. If these subsequent trials have failed, then IUI and fertility drugs are used to get pregnant.

iui treatment in india

IUI or Intrauterine Insemination is the next step of management in unexplained infertility

Intrauterine Insemination is a procedure which involves injecting or inseminating processed sperm into women's uterus around the time of ovulation. The goal of IUI is to increase the number of sperm that reach the fallopian tubes, thereby increasing the chances of fertilization. IUI cycles are also co-ordinated in combination with fertility drugs so that they stimulate the ovaries and increase the count of eggs.

Before the procedure, a woman is evaluated for infection, hormonal imbalances and other structural problems. Insemination is done at the time of ovulation, usually within 24-36 hours after fertility medicines are given to stimulate ovulation. Semen collected from the partner is washed at the lab to separate seminal fluid. A soft catheter is used to deposit the sperm directly into the uterus at the time of ovulation. IUI process increases the number of sperm inserted in the uterus and thus increases the possibility of conception. If the sperm fertilizes eggs, then pregnancy happens. IUI is a simple procedure which takes only around 15 to 20 minutes.

The success rate of IUI depends upon several factors such as female partner's age, health condition, fertility drugs used and others. However, if you are trying to get pregnant and looking for testing or other options to increase fertility, you can get expert help at Srishti Hospital, Puducherry. Pros and Cons of Gynaecological Surgeries

The procedure for gynaecological surgeries has evolved a lot in the past few years. Today, most gynaecological surgeries are assisted with laparoscopy. Previously laparoscopy was used in the only diagnosis and tubal ligations but today this tool is used in several gynaecological surgeries. Starting from the removal of ectopic pregnancy, ovarian cysts or treatment of endometriosis, laparoscopy has been an effective treatment option for all these critical surgeries.

What are the pros of Laparoscopic-assisted gynaecological surgeries?

Unlike open surgeries, advanced gynaecological surgeries have several benefits.
  • Minimally invasive process- Laparoscopic gynaecological surgeries involve incisions, but the cuts are very small and the marks usually fade away within a short period of time. Being a minimally invasive process, it involves very little pain and discomfort.
  • Increases patient’s safety- Any gynaecological surgery that is laparoscopy-assisted involves little risks. Well, to be frank, it's not that the patient is completely in a risk-free zone, but when compared to laparotomy, the risk is less.
  • Faster recovery- Previously when women used to go through gynaecological surgeries, it used to take a lot of time to recover and get back to their familial or professional life. But laparoscopy gynaecological surgeries allow the patients to get back to normal life within a short period of time. Even the hospital stay is minimal.
  • Affordable- Gynaecological surgeries these days are affordable as the patient do not have to stay in the hospital for a prolonged period, thus reducing the bill amount.

What are the cons?

Though the modern day gynaecological surgeries are less risky and more effective, they still carry certain risks:
  • Uterine perforation is one of the potential complications that are associated with injury to surrounding blood vessels.
  • There is a chance of urinary tract and bladder injury. Risk factors for urinary infection may include pelvic organ Prolapse and malignant disease.

How Does Lifestyle Affect Fertility In Men?

When a couple is unable to get pregnant in spite of having unprotected intercourse for more than a year, it means that either of the partner or both are going through infertility issues. There is a myth that infertility is only applicable to women. But it's the biggest lie ever! Around 30% of the fertility problems arise from men. It mainly happens due to abnormal or lack of sperm production.

If noticed very minutely, a few decades back the rate of male infertility was quite less. With each passing year, infertility in men is rising at a fast rate. But why? This is because of the kind of lifestyle one leads today. What biological disorder leads to male infertility?
  • hormonal problems
  • obstruction to the passage of sperm
  • Low sperm count and quality
  • Ejaculation dysfunction (impotence)

What lifestyle choices bring infertility in men?

Healthy sperms aren’t always gifted. It is one’s lifestyle choices that make the difference.

Following are some lifestyle habits that affect male infertility:
  • Smoking and alcohol consumption- Next to cancer, the second most common health issue created by tobacco and alcohol is infertility. The toxins inhaled from smoking tobacco are harmful enough to damage the quality of sperm. Further, smoking for a prolonged period of time creates a hormonal imbalance in the body which reduces the quality of the sperm produced.
  • On the other hand, alcohol consumption lowers testosterone levels and hampers the quality of the sperm produced. Excessive consumption can further reduce libido and cause impotency.
  • Stress- Psychological stress is the next harmful thing for sperm. Excessive stress brings hormonal imbalance in the body which results in low sperm count and quality.
  • Eating unhealthy food- Unhealthy or junk food does not have enough nutrients that can trigger the growth of new sperms. Wholesome foods like fish, brown rice, whole grains, eggs have a high quantity of zinc in it which maintain proper hormonal balance and production of sperm.

Male infertility is not a permanent problem. If you choose the right clinic for treatment, you can overcome infertility.

Benefits of IVF over other treatments

Almost 7 out of 10 couples worldwide are facing infertility issues. Some reasons are known whereas some are unknown. One of the most common reason is blocked fallopian tube and PCOS.

IVF or In-Vitro Fertilization has been a very successful method for couples with these kinds of fertility issues. It is the process in which egg cells are fertilized by the sperm cells outside the body, ie, in the laboratory. The treatment involves controlling the ovulatory process, drawing the eggs and the sperm from the couples and then fertilize them in a fluid medium. After the resulting embryo is formed it is passed directly into the woman’s uterus.

Why IVF is better than other assisted reproductive technology?

Before visiting a fertility Centre in Pondicherry, here are some things you should know about IVF.

IVF was introduced in 1978 and since then it has helped millions of couples to give birth to a baby. When other reproductive assisted technologies have failed in some cases, IVF has worked successfully. Here are some reasons why IVF is so popular:

  • IVF can be used by any couple. It can be used by surrogate mothers, same-sex couples or women who are unable to carry a baby full term.
  • In the case of IVF, a couple can use donated egg or sperms to achieve better results. Using a better quality egg or sperm increases the chance of getting pregnant.
  • Sometimes more than 1 embryo is formed through this process. If one of them fails to grow, others are kept reserved.
  • A genetic screening test is conducted through the IVF process. It increases the chances of a healthy baby. The genetic test also reduces the chances of miscarriages.
  • As multiple cycles can be used in this treatment, the chances of getting pregnant increases and live birth increases.
