Laparoscopic assisted Vaginal Hysterectomy (LAVH)

Vaginal Hysterectomy is a surgical procedure in which the uterus is removed through the vagina. In some conditions the fallopian tubes and ovaries are also removed.


The most common medical conditions for doing a hysterectomy includes the following:

What is Laparoscopic assisted Vaginal Hysterectomy (LAVH)? 

LAVH is a surgical procedure which uses a laparoscope to guide the removal of the uterus through the birth canal (Vagina). Whereas the procedure called a laparoscopic hysterectomy is performed by using a laparoscope and other instruments inserted through tiny abdominal incisions and the uterus, fallopian tubes etc are removed in tiny portions.

In some cases, a laparoscopic hysterectomy may be sufficient and in other cases and abdominal hysterectomy or a vaginal hysterectomy without a laparoscopy is indicated. The surgeon determines the procedure for the patient based upon the reason for hysterectomy and the condition of the patient.

How is LAHV performed?

In this procedure of LAVH slender metal tubes so called Trocars are inserted to provide passage for a laparoscope and other micro-surgical instruments by making several small incisions in the abdominal wall . The camera which is attached to the laparoscope provides images continuously and that is projected and magnified onto a television screen for viewing.
During LAVH, the uterus, the Fallopian tubes (if to be removed) is detached from the ligaments that attach it to other structures in the pelvis using the laparoscopic tools. Then the organs and tissue are removed through an incision made in the vagina.

The advantages of LAVH includes the following:

  • The incisions in LAVH are relatively small.
  • The scars, pain and recovery time are comparatively less than with an abdominal hysterectomy
  • LAVH is less physical traumatic

IVF or In Vitro Fertilisation is nothing but ‘Conception in a Lab’. It is the scientific method of getting a woman impregnated when some kind of obstacle to conception exists and was not overcome by the other assisted reproduction methods. It works by getting the egg fertilized with a sperm under tested laboratory conditions in a test tube. Procedure of IVF

The procedure involved depends on the kind of reproductive technology employed and whether donor sperms, eggs or embryos are involved. This may also be altered by another physical change of the body. 


The process usually starts with the hyperstimulation of the ovary medication which can either be by the short protocol or the longer one. The latter deals with the use of fertility medications to induce the ovaries to develop multiple follicles. On the other hand, the longer protocol makes use of a GnRH to downregulate the pituitary-ovarian axis and after a week the ovary is hyperstimulated with FSH. 

Once it reaches the desired state, egg retrieval would be scheduled. At the same time, semen would be retrieved. Fertilization would happen when the egg is placed in a special culture. The healthiest of the embryos is selected and is transferred to the uterus.

Pregnancy Care for Patients with Hypertension

The disorder of Hypertension in Pregnancy is a major cause of maternal, fetal and neonatal morbidity and mortality. Hypertension is the most common medical problem in pregnancy.

Women with hypertension in pregnancy have a higher risk of complications such as:
  • Abruptio placentae.
  • Cerebrovascular accident.
  • Disseminated intravascular coagulations.
The fetus has an increased risk of:
  • Intrauterine growth restrictions.
  • Prematurity.
  • Intrauterine death.
Hypertension which occurs during pregnancy are:
  • Gestational hypertension - Pregnancy-induced hypertension which develops after 20 weeks of gestation. It may be either transient hypertension of pregnancy or chronic hypertension and it cannot identified until the latter half of pregnancy.
  • Pre-eclampsia- Pregnancy-induced hypertension along with proteinuria or oedema or both.

Management will mainly depend on the woman's BP, gestational age and blood flow in the placenta. Non-pharmacological management is recommended for most of the women but is highly not recommended when there is the presence of any kind of associated maternal and fetal risk factors. Non-pharmacological management will include close supervision, limitation of activities and some bed rest in the left lateral position.
